Registration Requirements


Age Requirements


Pre-Kindergarten 3 students must be 3 years old on or before September 1st.

Pre-Kindergarten 4 students must be 4 years old on or before September 1st.

Kindergarten students must be 5 years old on or before September 1st.

Required Documents


Please submit the following items for review.

  • A completed Admission Application for each child.
  • Copy of the child’s Original/Certified Birth Certificate (not hospital certificate).
  • Copy of the child’s Baptismal certificate for any religion, if not baptized please note this on your application.
  • Copy of a recent Immunization Record.
  • Pastor Verification Form for families who are not members of Saint Teresa of Avila Parish.
  • For transfer students, grades K – 8, a copy of latest standardized tests from previous school must be included.
  • For transfer students, grades 1 – 8, a Release of Records Form.
  • For transfer students, grades K – 8, a Discipline Record.
  • A $75 per family non-refundable registration fee is due when admission application is submitted.

Transfer Students


For transfer students in any grade, you must provide a copy of the student’s most current medical, eye and dental records.  NEW exams, as shown below, are required for all students entering the following grades:

  • PreK 3 & 4  Medical* with the Lead Screening portion completed
  • Kindergarten Medical* with the Lead Screening portion completed, Vision/Eye, and Dental
  • Grade 2 Dental
  • Grade 6 Medical* and Dental

* Parent fills out and signs Health History portion on side two of the medical form
